Outpatient Internal/Family Practice Physicians.
· Has excellent clinical skills.

· Is licensed to practice medicine in the state of Arizona.
· Is board certified or board eligible in their specialty.
· Demonstrates a work history of positive collegial relations with colleagues, support staff, hospital-based providers, administrators and patients.
· Demonstrates advanced competency in their specialty.
Schedule/Availability Requirements:
Mon, Tue, Thur, Fri 7:30-5:30
Facility Location: 474 N Highway 89, Chino Valley
Setting: Outpatient Clinic
Duties: Family Practice
Days per month: 20
Required Procedures: toenail removals, lesion removals, Pap Smears, Joint injections. (negotiable)
EMR: Cerner
Support Staff: Medical Assistant/Triage RN/Front Office Staff/Clinic Manager
Reason for Coverage: Provider retirement
Deal breakers: Must be willing to do controlled substances for our Pain patients
